I would think that it's a bit strange that the Sega Gamepad Settings screen is parsing all the inputs accurately and lighting up next to each button as expected if it's an issue with the controller though right?

I would say it's strange that it works well here and with many other users with the exact same version ;-)
So I guess it might be a controller issue.

You could also try to connect the gamepad expander alone to the NerdSEQ to see if it works better.
Are you sure it is not the 'official sega' retrobits controller but a real original. I would definitely try also another one. And never hotplug either.
